Service Name: Welcome Home Service

 

Brief description of service:

The service offers a one-off visit to older adults living in Waltham Forest after discharge from Whipps Cross Hospital. We settle the older adult in at home and provide shopping, pension collection and small household tasks, if required. A follow-up visit can be made if there are concerns about the patient.

 

Times available/open:

Referrals can be made between 8.30 a.m. to 4.30 p.m. Monday to Friday

Referrals should be made by 3.00 p.m. one day prior to service being required.

 

Languages available:

English and Asian community languages.

 

How to access service:

Referrals are only accepted from staff at Whipps Cross Hospital.
