What we do

The initial self assessment questionnaire identifies older people who have fallen or who are at risk of falls, older people that might be beginning to have problems such as, memory loss, depression, functional decline and issues with medication as well as social isolation.

Once the information provided has been analysed those identified as in need of significant intervention are contacted for a single assessment interview.  We will arrange to visit you at home and all aspects of daily life will be assessed to identify where there may be needs and what solutions can be offered. Please note that we never visit without having made an appointment with you and we always have an identity badge that you should ask to see.

Assistance offered may take the form of commissioning minor adaptations to your home or equipment relevant to needs. The assessor may refer you for a further assessment (a specialist assessment) or refer you to another Age Concern Waltham Forest service such as our Information & Advice service whose staff have expertise about benefits you may be entitled to.

Following any assessment, we will write to you giving the details of what we agreed may be helpful and then we will follow up to check that everything we agreed has been put in place.  

You are under no obligation to accept any equipment/adaptations or referrals that may be suggested and information provided to us is confidential.

Our team consists of an Asian speaking assessor if this would be appropriate for you.

Even if you do not have significant needs, and do not need an assessment, you will receive information that may assist you to continue to live independently at home.

I am the lead of the Age Concern Waltham Forest, Crisis Prevention service.

Since 2002 we have been undertaking preventative single assessments on behalf of Waltham Forest PCT and Social Services. As an extension of this work, we are now able to offer the service to older people on-line.
