Brief description of service:

A dedicated service for isolated older adults with a diagnoses of a terminal illness.  We supply an experienced sessional worker who visits weekly for 2 hours building a close relationship and offering support, including outings.

Case Study

H was referred to the Link-Up support service in February ’05, by the MacMillan nurses.  She was unable to take up the service immediately as she was too ill, suffering with cancer of the vocal cords.  

H was introduced to her sessional worker in May ‘05 and has continued to enjoy her visits.
