Hospital discharge
We run a number of services for older adults who are being discharged from Whipps Cross University Hospital. Older adults in Whipps Cross University Hospital should ask to speak to one of the Age Concern Waltham Forest staff if they wish to receive any of the these services or if they may need other forms of support on leaving hospital.
A&E escorted discharge service
The A&E Escorted Discharge Service provides a service to older adults, living in the Waltham Forest area, who are being discharged from A&E and would go home with a relative, if available. The service also settles the older adult into his or her home surroundings and will do shopping, pension collection and minor housework tasks.
Discharge lounge project
The project aims to identify older adults being discharged from hospital and address any concerns this person may have about their discharge or coping at home, and to make the transition from hospital to home as early and comfortable as possible. As part of the project we also trying to identify socially isolated older adults and fallers, who will be referred to other Age Concern Waltham Forest services. The Discharge lounge project is based in the Discharge lounge.
Welcome home service
The Welcome home service provides a visit to older adults after discharge from hospital. We provide setting in, shopping, pension collection and small household tasks.
Ward scheme
Age Concern Waltham Forest has a presence in some of the Whipps Cross University hospital wards. Staff are visiting older adults and aim to identify those who might need community based support services after discharge from hospital. We try to identify older adults at risk of social isolation or fall and aim to provide the person with information and related services.
